Depression Screening

Have you or a loved one been experiencing a recent change in mood?  Has this change been interfering with your daily activities? 

 

Changes in mood and behavior may be due to clinical depression.  If so, effective treatment is readily available.  To help understand the symptoms involved in depression, answer the questions below. 

 

Please note that the following is a brief depression screen.  It is not a substitute for a comprehensive clinical evaluation and should not be used to make a formal diagnosis.


Over the past two weeks, have the following been significant problems for you:

1. been feeling low in energy, slowed down?
2. been blaming yourself for things?
3. had poor appetite?
4. had difficulty falling asleep, staying asleep?
5. been feeling hopeless about the future?
6. been feeling blue?
7. been feeling no interest in things?
8. had feelings of worthlessness?
9. had difficulty concentrating or making decisions
10. thought about or wanted to commit suicide?**

How many “Yes” answers are recorded above?  If you answered “Yes” to three or more of the questions, then it is recommended that you meet with a mental health professional to evaluate the possibility that your distress is being caused by depression.  If you answered “Yes” to five or more of these questions, then such an evaluation is strongly recommended.
